On the transformation tab we need to select all the fields in the left and right hand side columns. We then delete any mappings that have been create already by SQL Server. Create a new transformation of the type ActiveX Script. Here is the page I saw when I was building this demonstration package.

Although you can see the designer has mapped the first column in the source to the first column in the destination there is nothing for our author's first name or last name. Take a look over in the left hand side of Figure 1.5 and you'll see our lookup we just created.
